Making a Contribution
to Support Alzheimer's Services
 
 
We welcome contributions that are used to support the many programs and services of Alzheimer's Services.
These contributions are acknowledged to the donor with a letter that can be used for tax purposes.  (list of Programs and Services) 
  
You can contribute by mail or online. 

To make your donation by mail, please print out our on-line form and send your contribution to our office.  Our address is:

Making a difference in the lives of thousands of Northern Indiana Alzheimer families is only a click away. 
 
If you would like to make a credit card contribution on-line, please click the e-mail icon below and send us a message which includes your 1) name, 2) address, 3) credit card number (we can only accept Master Card and VISA), 4) expiration date, and 5) the amount you wish to donate.  For security reasons, credit card numbers are not stored on any of our databases.
