Making a Contribution as a Memorial/Tribute    

We often receive contributions in honor or in memory of someone.  These contributions are acknowledged to the donor with a letter that can be used for tax purposes.     We will also send a letter to the person honored or family members of the individual memorialized to let them know of the contribution.  We do not include information regarding the size of the gift when we send a notification letter.     You can contribute by mail or online.     To make your donation by mail, please print our on-line form and send your contribution to our office.  Our address is:  Alzheimer's Services  922 E Colfax Ave South Bend, IN  46617 If you would like to make a credit card contribution on-line, please click the e-mail icon below and send us a message    which includes your 1) name, 2) address, 3) credit card number (we can only accept Master Card and VISA), 4) expiration date, 5) the amount you wish to donate, 6) the name of the person being honored/memorialized, and 7) the name(s) and address(es) of the person(s) to be notified of the contribution.  For security purposes, credit card numbers are not stored on any of our databases.       Our web site is not currently secure.  Therefore, if you are  not comfortable making an on-line contribution please call  our office at (219) 232-4121, or toll-free (888) 303-0180 and provide the above information.                            Through generous friends like you, we are increasingly closer to achieving our goal of a world without Alzheimer's  disease.
